Welcome! Our child and adolescent psychiatry practice warmly welcomes patients who speak English, Russian, Greek, or Arabic. We are dedicated to offering compassionate support and personalized care, creating a space where everyone feels safe, understood, and respected.

Here you will find the registration forms for an initial appointment in our practice. We kindly ask you to answer each question to the best of your knowledge and return the form to our office (online, via email, fax, or mail). We will contact you as soon as possible to schedule an appointment. Please also note that we only treat patients with private health insurance in our practice. However, if you wish to be treated in our practice but do not have private health insurance, you can cover the costs yourself.
